Job Description:
A company is looking for a Community Health Worker to conduct
member outreach and promote healthy outcomes for members in North
Carolina.
Key Responsibilities
Conduct screenings of member health care needs and develop
healthcare goals
Utilize motivational interviewing techniques to engage members in
positive behavior change
Coordinate access to health-related programs and provide education
on health and wellness
Required Qualifications
High school education or equivalent experience
Experience within the healthcare industry
Proficiency in Microsoft applications, including Excel, Word, and
Outlook
Ability to work independently and as part of a team
No professional license or certification required
